Transition services create a bridge between school and successful adult living. The program collaborates and contracts with schools statewide to help support students achieve their goals. The program was built from the ground up at Hands of Heartland. Under the direction of a licensed Special Education teacher, a specially trained coach works with individuals and their families to provide an individualized program best suited to that young person’s needs, strengths, preferences, and interests. This time alongside passionate facilitators and peers creates growth opportunities that prepare students for their bright futures.
