Family and Peer Mentoring

This service provides mentors who have shared experiences with families or participants. The mentors help individuals and families navigate the system and build relationships that become cherished and valuable connections. Mentors provide support, guidance, and care in the process by sharing strategies, personal experience, resources, and possibilities with families, care givers, and individuals receiving services. They want to offer solutions and help eliminate some of the confusion or guess work when exploring all opportunities for care.

Other Family Support Waiver Services

Transportation
Facilitates rides for individuals traveling to and from waiver services – separate from transportation naturally happening (to an outing, volunteer opportunity) during service
Supported Family Living
Guidance from a care provider in the family home that increases independence, community connection, and confidence as the individual practices daily living skills
