A specially chosen mentor helps families navigate the system and tap into available resources for their loved one to achieve best outcomes
Facilitates rides for individuals traveling to and from waiver services – separate from transportation naturally happening (to an outing, volunteer opportunity) during service
Guidance from a care provider in the family home that increases independence, community connection, and confidence as the individual practices daily living skills
