The Transition Program is designed to support individuals (ages 12-21) with intellectual and developmental disabilities (I/DD) that carries them from school into adult life.

We partner and contract with schools to help students feel supported and reach their educational and personal goals. Transition is a collaboration between Hands of Heartland, Nebraska schools, families, and students to create an educational experience designed for the needs of each student.
With the guidance of an experienced teacher, staff and students learn and grow together. Each person’s hobbies and interests are included in their specific curriculum to create exciting, fun outcomes and pathways to success.
The primary focus is building confidence and preparing students for greater independence, connections, and community integration through a person-centered approach.

Education Partnership
Each student’s program is tailored to their individual interests, strengths, and goals. Regular meetings and assessments ensure that the services provided align with their evolving needs and aspirations.
Daily Living Skills
The program emphasizes essential life skills such as money management, cooking, budgeting, laundry, hygiene, and household chores. This training helps students develop the competencies needed for daily living and independent functioning.
Community Experiences
Students participate in various community activities, like volunteering at local organizations like Meals on Wheels and the Children’s Museum. These activities foster a sense of purpose, belonging, and social interaction, which are crucial for their personal development.
Employment Skills
Students’ job goals are explored and they can receive guidance on job searching, resume building, and interview preparation. The goal is to help them secure and maintain meaningful employment in a supportive work environment.
